Changes in insurance plans and participation occur frequently. We accept most major insurance plans. If you do not see your insurance listed above or if you are not sure if we accept your plan, please do not hesitate to contact us. We will be glad to verify your physical therapy benefits.
We do accept out-of-network benefits should this be provided under your insurance plan.
Terms of Service
Patients are responsible for any co-payment, deductibles and/or any coinsurance payment due at the time of service. For your convenience, payments can be made by cash, check, or credit card. As a service to our patients, Ergo Physical Therapy offers the following assistance:
- Contact your insurance company directly to verify your benefits, copayment and/or coinsurance
- File your insurance claims when provided with the required insurance information
- Monitor insurance authorizations, number of authorized visits and prescription expiration date
- Fax initial evaluation, progress and discharge summaries to your physician prior to follow up appointments
For your convenience, please visit our Patient’s Center. You may download all forms necessary for your initial evaluation appointment. To ensure faster processing time in the office please fill out these forms prior to your office visit and bring them with you. Please note that this information must be accepted at the front desk PRIOR to your initial evaluation.
